Parents as you know when school begans in Aug. your child will be a 7th grader. As a 7th grader they must have two booster shots-a second Varicella or history of chicken pox and Tdap booster(tetanus-diphtheria-pertussis). At the time of the shots the DR's office will give you a Certificate of Immunization form, this form will need to be brought to school to be placed in their permanet school record.
